Summary

PREPARETM for your care (prepareforyourcare.org) is a didactic in the form of a series of documents and film clips which are intended to instruct the viewer how to make medical decisions and document them so they can be realized at a later time.

This tool was developed by a team led by Rebecca Sudore, MD, FAAHPM, Professor of Medicine, Geriatrics Division, University of California, San Francisco.

From the website: "PREPARE for Your Care is an online resource that helps people learn about and prepare for medical decision making.

This evidence-based tool features video stories in English and Spanish and guides users as they explore their wishes and learn how to discuss them with family, friends, and medical providers. The website also offers PREPARE written pamphlets as well as a Toolkit to help put on a PREPARE Group Movie Event. These Movie Events can be used in group medical visits or in the community. PREPARE also offers easy-to-read, legally-binding advance directives for all 50 states in English and Spanish.

The PREPARE website walks people step-by-step through questions about what is most important in life using video stories. Going through the PREPARE website and seeing the video stories is the best way to help you answer these questions. But, you can also get a written copy of the PREPARE Question Guide here."

This is one of hundreds of products and resources in the Tech-enhanced Life database of "useful products and resources".

The theme of this collection of resources is "tools we can use to help improve our quality of life as we grow older".

We have no vested interest in any of these products or services, but we have collected descriptions of them, and organized them according to various themes relating to the challenges of aging — so older adults and their families can find useful products they do not yet know about.
