Five wishes is an online service that enables you to create an advance directive.
The advance directive created using Five Wishes includes the normal healthcare-related components one finds in advance directives from other places, but also includes personal and spiritual wishes, which not all advance directives do.
They have online and downloadable versions, and some guides and resources to help make decisions and interact with family members.
This is a paid service, unlike many of the other advance directive online resources which are free. However the fees are modest. While you need to pay to get the final form, you can download a sample to get a feel for how it works.
